TERMS AND CONDITIONS OF SERVICE

Introduction and Agreement

These Terms and Conditions ("Terms") govern all treatments, consultations, products, and services provided by Sculpt & Revive Aesthetic Clinic ("the Clinic," "we," "us," or "our"), registered at 263 Walworth Road, London, SE17. By booking an appointment, attending a consultation, or receiving any treatment, you ("the Client," "you") confirm that you have read, understood, and agree to be bound by these Terms in full. These Terms constitute a legally binding agreement. If you do not accept these Terms, you must not book or proceed with any treatment.

About the Clinic

Sculpt & Revive Aesthetic Clinic is a non-surgical aesthetic clinic based in South London, accredited by Save Face, the national register of accredited practitioners in aesthetic medicine. Our treatments include:

•       Anti-wrinkle injections (botulinum toxin / Botox)

•       Dermal fillers (lip augmentation, cheek enhancement, facial contouring, and other areas)

•       Laser hair removal

•       Laser skin treatments (pigmentation, resurfacing, and rejuvenation)

•       Facials and bespoke skin treatments

•       Microneedling (including with serums and skin-boosting agents)

•       Skin boosters and hydration treatments (Profhilo, polynucleotides)

•       Chemical peels

•       Other non-surgical aesthetic procedures as updated from time to time

All injectable treatments involving prescription-only medicines (POMs), including botulinum toxin and dermal fillers, are administered in accordance with current UK prescribing regulations. A qualified prescriber will assess your suitability prior to treatment.

Client Eligibility

3.1 Age Requirement

All treatments are strictly available to clients aged 18 years or over. We will request proof of age where there is any reasonable doubt. It is unlawful to perform certain aesthetic treatments on persons under 18 and the Clinic will not proceed under any circumstances.

3.2 Accurate Information

You must provide accurate, complete, and truthful information at all times including: full name, date of birth, and contact details; complete medical history and current medications; history of previous aesthetic treatments; known allergies or sensitivities; and pregnancy or breastfeeding status. Providing false or incomplete information may result in treatment refusal or may increase your risk of adverse events, for which the Clinic accepts no liability.

Consultations and Treatment Suitability

4.1 Mandatory Consultation

All clients must complete a consultation prior to receiving any treatment. Consultations assess suitability, review medical history, discuss goals and expectations, explain procedures and associated risks, and obtain informed consent.

4.2 Prescriber Assessment (Injectables)

In accordance with UK law, all prescription-only medicines including botulinum toxin require assessment by a qualified, registered prescriber, either in person or via a valid remote consultation. Treatment will not proceed without a valid prescription.

4.3 Right to Decline Treatment

The Clinic reserves the absolute right to decline, postpone, or discontinue any treatment at its sole discretion, including where a client is clinically unsuitable; required consultations or consent forms have not been completed; the requested outcome cannot be achieved safely or ethically; a client attends under the influence of alcohol or controlled substances; or the Clinic has reasonable concerns regarding a client's mental health or capacity to consent.

Where treatment is declined on clinical grounds, any deposit paid will be refunded in full.

Treatment-Specific Terms and Conditions

5.1 Anti-Wrinkle Injections (Botulinum Toxin)

Botulinum toxin is a prescription-only medicine. Treatment is carried out only following a prescriber assessment in line with MHRA and JCCP guidance. You must not be pregnant or breastfeeding and must disclose all medications including blood thinners, antibiotics, and supplements. Avoid alcohol for 24 hours before treatment. Results typically appear within 3-14 days and last 3-6 months. Repeat treatments are required to maintain effect. No specific outcome is guaranteed. A complimentary 14-day review appointment is offered exclusively for anti-wrinkle injection treatments. This review is provided to assess the result and, where clinically appropriate, to administer a minor top-up at the practitioner’s discretion. The 14-day review does not apply to any other treatment. Any removal, reversal, or dissolution of botulinum toxin effect is not included within the scope of the 14-day review and will not be carried out unless the client requests this as a separate chargeable treatment. The Clinic reserves the right to charge for any reversal or removal procedure, and no such procedure will be performed without the client’s prior agreement to the applicable fee.

5.2 Dermal Fillers

Dermal fillers (typically hyaluronic acid-based) carry risks including bruising, swelling, infection, lumps, asymmetry, and in rare cases vascular occlusion - a serious complication that can lead to tissue damage or, in very rare cases, visual impairment. You must disclose all previous filler treatments and any complications, and disclose any history of herpes simplex prior to lip treatments. You must not have received dental work within 2 weeks before or after treatment. In the event of any post-treatment complication, contact the Clinic immediately. Emergency dissolving with hyaluronidase is available where clinically indicated. Do not seek treatment from third parties for complications without contacting the Clinic first.

5.3 Laser Hair Removal

A patch test is mandatory at least 24-48 hours before your first full laser hair removal session. You must avoid sun exposure, tanning beds, and self-tanning products for at least 4 weeks before and throughout your treatment course. Shave the treatment area 24 hours prior to each session. Do not wax, thread, or epilate during the course. Disclose all photosensitising medications. Multiple sessions are typically required (6-12). Results are not guaranteed. All laser treatments are performed by Level 4 laser-qualified practitioners.

5.4 Laser Skin Treatments

Laser skin treatments carry risks of redness, swelling, blistering, and in some cases changes to skin pigmentation, particularly in clients with darker skin tones (Fitzpatrick types IV-VI). A patch test is required. Disclose active skin conditions, history of keloid scarring, photosensitising medications, and any recent sun exposure or planned sun holidays.

5.5 Facials and Skin Treatments

Facial treatments are tailored to your skin type and concerns. Disclose active skin conditions, known allergies to skincare ingredients, and any recent treatments including peels, laser, or injectables within the past 2-4 weeks. Some treatments may not be suitable during pregnancy.

5.6 Microneedling

Microneedling is contraindicated where a client has active acne, rosacea, eczema, or psoriasis in the treatment area; a history of keloid or hypertrophic scarring; blood clotting disorders or anticoagulant medications; or active skin infections. Redness and sensitivity are expected for 24-72 hours post-treatment. Multiple sessions are required and results emerge gradually over several weeks.

5.7 Chemical Peels

Chemical peels carry risks of redness, peeling, sensitivity, and in rare cases pigmentation changes or scarring. A patch test may be required. Disclose all current skincare products including retinoids and acids, recent sun exposure, and any skin conditions. Daily SPF 50 sunscreen is mandatory following all peel treatments.

Informed Consent

Prior to every treatment, you will be required to complete a consultation questionnaire and sign a written informed consent form. By signing, you confirm you have been informed of the procedure, expected outcomes, risks, and alternatives; have had the opportunity to ask questions to your satisfaction; consent freely and without pressure; and understand that results are not guaranteed. Consent may be withdrawn at any time before treatment commences. The Clinic maintains records of all consent forms in accordance with clinical record-keeping requirements.

Medical History and Disclosure

You must disclose your full and accurate medical history prior to treatment, including: all current medications, vitamins, and supplements; known allergies or adverse reactions; previous aesthetic treatments; autoimmune conditions, bleeding disorders, or immunosuppression; pregnancy or breastfeeding status; history of cold sores or herpes simplex; history of keloid or hypertrophic scarring; active skin conditions including eczema, psoriasis, acne, or rosacea; and mental health conditions including body dysmorphic disorder (BDD).

Failure to disclose relevant medical information may increase your risk of adverse effects. The Clinic accepts no liability for complications arising from undisclosed conditions or medications.

Aftercare and Post-Treatment Obligations

You will receive written aftercare instructions following every treatment, which you must follow carefully. Failure to do so may affect your results or lead to complications for which the Clinic accepts no liability. Standard aftercare includes avoiding sun exposure and extreme heat as advised; not touching the treated area unless instructed; applying SPF 50 daily following laser or skin treatments; and promptly reporting any adverse reactions or complications to the Clinic.

If you experience any of the following after an injectable treatment, seek immediate medical attention and contact the Clinic without delay:

•       Severe or worsening pain

•       Blanching, mottled, or discoloured skin

•       Vision changes or eye pain

•       Signs of infection: heat, swelling, pus, or fever

Bookings | Deposits | Payments

9.1 Booking Confirmation

Appointments are confirmed upon receipt of written or electronic confirmation from the Clinic. We reserve the right to decline or cancel bookings at our discretion.

9.2 Deposits

A non-refundable deposit may be required to secure your appointment. Deposits are deducted from the total treatment cost on the day of your appointment. Deposits are forfeited in the event of a late cancellation or no-show. Deposits are refundable in full only where treatment is declined on clinical grounds.

9.3 Pricing

Prices are as displayed on our website or price list at the time of booking. We reserve the right to amend prices at any time, with changes not affecting existing confirmed bookings.

9.4 Payment

Full payment is required on the day of the appointment, prior to or at the time of treatment. We do not offer credit or deferred payment plans unless specifically agreed in writing in advance.

Cancellation | Rescheduling | No-Shows

10.1 Notice Required

A minimum of 48 hours' notice is required to cancel or reschedule any appointment. For laser courses or combination appointments, 72 hours' notice may be required as communicated at booking.

10.2 Late Cancellations

Cancellations within the required notice period may result in forfeiture of any deposit paid. The Clinic reserves the right to charge a cancellation fee of up to 50% of the treatment value.

10.3 No-Shows

Failure to attend without notice results in forfeiture of any deposit paid. Repeated no-shows may result in the requirement for full prepayment for future bookings or refusal of future appointments.

10.4 Rescheduling

Appointments may be rescheduled free of charge with at least 48 hours' notice, subject to availability. Deposits are transferable to a rescheduled appointment where sufficient notice is given.

10.5 Clinic-Initiated Cancellations

In the event of a clinic-initiated cancellation, you will be offered an alternative appointment at your convenience or a full refund of any deposit paid. The Clinic is not liable for any costs you have incurred in connection with the cancelled appointment.

Late Arrivals

Please arrive on time. Arriving more than 10 minutes late may result in your appointment being shortened or cancelled and treated as a late cancellation. Full charges apply regardless of time lost due to late arrival. The Clinic will not compromise treatment quality or safety by rushing a procedure.

Treatment Results and Expectations

Results vary between individuals based on age, skin type, metabolism, lifestyle, and adherence to aftercare. The Clinic does not guarantee any specific outcomes. Dissatisfaction with results does not constitute grounds for a refund where the treatment has been correctly administered. Clinical photographs may be taken for record-keeping. Separate written consent is required before photographs are used for promotional or marketing purposes.

Refund Policy

13.1 Completed Treatments

Due to the nature of aesthetic treatments, refunds are not provided for treatments that have been administered. By proceeding with treatment, you accept this policy.

13.2 Treatment Packages

Unused sessions within a purchased package may be refunded at a pro-rata rate (based on single-session pricing) at the Clinic's discretion, minus any applicable administration fee.

13.3 Products

Skincare products may be returned unused and unopened within 14 days for an exchange or store credit. Opened products may not be returned unless faulty.

13.4 Clinical Negligence

Nothing in these Terms limits your right to pursue a complaint or claim where you have suffered harm as a result of the Clinic's negligence or breach of duty.

Patch Tests

A patch test is mandatory prior to laser hair removal, laser skin treatments, and may be required for chemical peels and certain other treatments. Patch tests must be completed at least 24-48 hours before the full treatment. The Clinic accepts no liability for adverse reactions where a patch test was not completed due to the client's failure to attend.

Limitation of Liability

15.1 General Limitation

The Clinic's liability to you in connection with any treatment or service shall be limited to the direct cost of that treatment or service. The Clinic shall not be liable for any indirect, consequential, or economic loss.

15.2 Excluded Liability

The Clinic accepts no liability for adverse outcomes arising from: failure to disclose relevant medical conditions, allergies, or medications; failure to follow pre- or post-treatment instructions; normal expected side effects disclosed during consultation; results that differ from other clients or promotional materials; or third-party treatment following your treatment with us.

15.3 Statutory Rights Preserved

Nothing in these Terms excludes or limits the Clinic's liability for death or personal injury caused by its negligence, for fraud or fraudulent misrepresentation, or for any other liability that cannot be excluded or limited by applicable law.

Health and Safety

The Clinic operates in accordance with all applicable health and safety legislation and relevant infection prevention and control standards. Inform us of any changes to your health or medications between bookings. Do not attend the Clinic if you are unwell, have an active infectious condition, or are in the early stages of a cold sore or skin infection. All equipment is cleaned, disinfected, or sterilized between clients in accordance with applicable guidelines.

We aim to acknowledge all complaints within 2 working days and provide a full written response within 14 working days. If you remain dissatisfied, you may escalate your complaint to Save Face (www.saveface.co.uk).

Data Protection and Privacy

The Clinic processes personal data in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018, for the purposes of providing and administering treatment, clinical record-keeping, appointment communications, and where explicit consent has been given, marketing. We do not sell or share your data with third parties except where required for your clinical care or by law. You have the right to access, correct, and request deletion of your personal data. Photography and Promotional Use

Clinical photographs may be taken before and after treatment for record-keeping purposes. These are stored securely as part of your clinical record. Explicit written consent will always be obtained separately before any images are used for promotional, marketing, or educational purposes. Consent for promotional use may be withdrawn at any time in writing.

Intellectual Property

All content, branding, trade marks, logos, photography, and materials owned by or licensed to Sculpt & Revive are protected by applicable intellectual property law. You may not reproduce, distribute, or use any such materials without prior written consent from the Clinic.

Amendments to These Terms

The Clinic reserves the right to amend these Terms at any time. Updated Terms will be published on the Clinic website with a revised effective date. Continued use of our services after such publication constitutes acceptance of the updated Terms.

Governing Law and Jurisdiction

These Terms are governed by and construed in accordance with the laws of England and Wales. Any dispute arising in connection with these Terms shall be subject to the exclusive jurisdiction of the courts of England and Wales.
